One Household Mobility Plan

Defines household goals, barriers, actions, responsible staff, timelines, resources, and intended outcomes.

The Integration Specialist

Protects the whole household picture across partners and prepares households for Mobility Council review.

The Household Mobility Council

Cross-agency representatives authorized to act, commit resources, and carry out one Household Mobility Plan.

Shared Household Outcome Measurement

Measures progress toward stability, mobility, reduced crisis use, income growth, and economic independence.

Economic Independence

The ultimate household destination: increased earned income, reduced dependency, asset ownership, and financial resilience.
